Every patient in recovery must have a specialized, individualized program that takes into consideration different addictions, different substances, differing commitment to recovery... eg: a drug court patient may not have the same motivation to be in treatment as a treatment-seeking patient. Treatment Centers that individualize each person's program is imperative so that the each individual's weaknesses are addressed and path to recovery is maximized and never compromised. 


Highly Individualized Program

Outpatient treatment should include ALL of the following:

Counseling

Regular evaluation by a medical professional 

Group support meetings (read12-steps is NOT for everyone and this article).

Meetings that include the family/support group and the person recovering. 

Meetings with the family/support group to educate them on the process of recovery and how to handle likely situations that will arise in the long course of recovery (both while receiving treatment, and after treatment has terminated).

Outpatient

Heroin has a 2-4 day detection period. The treatment should include frequent, routine testing that shows that the person is not only heroin-free, but is taking the medication prescribed to him/her.

Should Include Multiple Forms of Support

The obvious drug-free environment may not be as easy for the person recovering from heroin addiction as for those recovering from other addictions, because many sober living environments (SLE) do not allow medication-assisted-therapy (MAT). They have a "0 tolerance" for anything except cigarettes.This is a rigidity that may cost the person recovering from heroin addiction his/her life. Do not underestimate the importance of MAT

Out-patient treatment should be intensive (5 days per week) at least to begin with. This is a lifelong journey and different individuals will need  programs with different durations and intensity in order to avoid relapse.

Heroin is the only substance whose success of recovery DOUBLES for the addict if treatment medication is included in the recovery program (2). This SHOULD NEVER be viewed as "substituting one drug for another."
